Jon-Erik Hexum

While filming the television show Cover Up, Jon-Erik Hexum become bored between shots and unloaded a prop gun of all but one of the blanks and began a game of Russian roulette, not realizing that blanks can be deadly if fired at close range. He put the gun to his right temple and pulled the trigger, killing him instantly. He's pictured here in his earlier hit NBC-TV series, Voyagers! co-starring child actor Meeno Peluce.
